Supervisor of Case Management

This is a responsible, supervisory position providing direction to case management staff responsible for providing services to children and their families who are referred per the eligibility requirements identified by contract. Work involves coordination of team members' activities, monitoring of services and assurance of compliance with all rules and regulations regarding the child welfare system and contract compliance.

DUTIES

  • Ensures that reporting staff maintain clear and concise documentation for services within the established time frame and standards per statute, and Child Protection Community Based Care Policies and Procedures.
  • Provide complete documentation for services rendered within established time frames, including daily log, progress notes, assessments and treatment plans, and other required documentation.
  • Ensures that reporting staff complete client transfers, referrals, summaries, closings, reports, authorization for release of information and other forms according to Organization and Program policies and procedures.
  • Ensure that reporting staff maintain established caseload productivity requirements. Maintain established standards for direct client contact.
  • Ensure that reporting staff maintain client confidentiality according to established policies, federal and state statutes. See Performance Standards.
  • Ensure that reporting staff complete assigned paperwork in compliance with standards, including time sheets, mileage logs, leave request, client' records, client appointments and management information system.
  • Ensure that reporting staff provide quality services to clients and families as evidenced by assessment of services, client satisfaction surveys and the quality improvement system.
  • Ensure that reporting staff maintain client's contact information and evacuation plan in the event of a natural disaster and must have availability for contact for self during such natural disaster.
  • Ensure that all reporting staff complete documentation for closed records within 24 working hours of case closure approval and submit closed cases to assigned supervisor for review.
  • Ensure that reporting staff complete necessary documentation to comply with revenue maximization requirements including, but not limited to: treatment plan, IV-B, IV-E; IV-A and Relative Caregiver documentation. (Applicable for case transfers to child safety)
  • Ensure that reporting staff enter data/progress notes or other required information into state and AVATAR as required by the Child Protection Community Based Care Policies and Procedures and Family Preservation Policies and Procedures.
  • Convene team meetings on a regular basis.
  • Assign cases through case transfer process and Intake, assuring a balanced case load
  • Ensure that all required reports and paperwork to meet compliance with the contracting agency and other governing bodies are completed by required due dates.
  • Oversee the completion of client transfers, referrals, summaries, reports, authorization for release of information and other forms according to Organization and Program policies and procedures.
  • Review documentation for closing case records to ensure compliance with program policies and procedures.
  • Regularly attend scheduled Organization and Program meetings as appropriate.
  • Ensure that treatment plans are developed within the expected time frame with all appropriate signatures.

QUALIFICATIONS

  • At least four (4) years' experience with in human services or child welfare programs with a Bachelor's degree in social work or a related area.
  • Master's degree in social work or a related area with at least (2) years' experience in human services or child welfare programs.
  • Must be certified as a Child Welfare Professional.
  • Must possess a reliable vehicle to drive to do field work and maintain a safe driving record, maintain specific automobile insurance coverage as required by the organization and undergo an annual vehicle safety inspection if transporting clients.
