Student Care Manager

Student (youth) Care Managers will be available to families for emergency crisis intervention on all students open in the program regardless of risk level.  Directions for Living will provide an on-call rotation to ensure that all families being served in the Treatment Works program have access to a Student (youth) Care Manager  and/or a Youth  & Family Counselor to assist with stress management, crisis intervention, skills training, and problem solving. The Treatment Works Program Supervisor will be available by telephone at all times for emergency consultation. In the event the supervisor is unavailable, the administrator on duty will serve as second in command to the supervisor. For the first 12 months, services will primarily be in the home and out in the community with the Treatment Works Team traveling to meet the student or family where they are, thus eliminating a transportation barrier. Student Care Managers services includes assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, entitlement, and vocational rehabilitation to help students and families reach their goals, live and participate fully in their community.  It includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual, which reflects strengths and self identified goals.  As an integral part of Student Case Management services, discharge/transition services are arranged or coordinated by the Student case manager as the student and family is preparing for discharge and transition from one level of care to another.  These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training or school attendance.)  All services will be provided within a Recovery-oriented system of care, which builds on the student and family strengths and incorporates community, natural systems, and clinical supports and services in the management of the student and family's needs. Responsible to meet productivity requirements of 75% which includes 1440 billable hours annually or a minimum of 30 billable hours per week.

 

EDUCATION EXPERIENCE/REQUIREMENTS:

Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with high risk k-12 students and their families.

Must be eligible for Targeted Case Management Certification.

Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.

Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
