Senior Case Manager-Homeless Services

POSITION SUMMARY:

This is a responsible supervisory position providing leadership for programs providing prevention and diversion services, treatment and outreach in the community serving children, families, and adults.  It requires management and supervisory skills, and clinical expertise with prevention, diversion and co-occurring disorders.  This position assists in the supervision of homeless outreach and street outreach staff. Provides onsite supervision of case management staff at Pinellas Safe Harbor.

PRIMARY RESPONSIBILITIES:

Administrative/Supervisory Responsibilities

Assists Homeless Services Supervisor in ensuring productivity levels of staff are adequate and maintained on an ongoing basis per organizational standards.

Assists in the coordination and provision of services among the multiple providers at Pinellas Safe Harbor and in the community maintaining an excellent working relationship with all.

Assists Homeless Services Supervisor in keeping statistics and completing administrative reports and assists in monitoring corrective actions as needed.

Assists supervisor in monitoring progress and ensuring achievement of each service goals and objectives through the development and monitoring of an annual Quality Center Plan.

Participates in the process of program planning as appropriate.

Ensures that client records are maintained with compliance.

Assists Homeless Services Supervisor in the screening of applicants for   open positions.

Ensures the development of performance objectives for staff under supervision, and encourages participation of those employees in relevant training and educational activities.

Assists in maintaining a high level of performance, provides supervisor with feedback to assist in the staff evaluation process. Assists supervisor in monitoring corrective action process when necessary. 

Organizes work, establishes priorities and exemplifies adaptability to change and creative thinking to improve quality, productivity, and customer service/satisfaction.

Motivates staff; keeps staff well informed and solicits input to appropriately manage the program and improve customer satisfaction.

Confronts staff as needed; promotes professional responsibility; coaches staff to be successful in their job duties.

Demonstrate good work ethics, drive, enthusiasm and persistence to achieve program goals.

Encourages initiative, choice, growth and professional development among staff.

Delegates responsibility to staff and develops staff by improving their skills and competencies for current and future responsibilities.

Maintains clear boundaries with staff.   Treats staff with dignity and respect.

Handles difficult situations in a tactful manner and provides constructive feedback to staff on an ongoing basis and takes appropriate corrective action for sited deficiencies.

Assists supervisor in the preparation of report required by funders and organization.

Single point of contact for referrals and management of caseloads.

Clinical Responsibilities

Develops accurate and complete clinical evaluations of client.

Insures the development and updates of realistic treatment plans with clients and their families as required and ensures that all documentation exhibits sound clinical judgment, identifies client progress, strengths and issue and documents client participation in the service process.

Conducts thorough client interviews exhibiting accuracy and thoroughness of assessment and referral skills.

 Ensure staff manages caseload appropriately, establishes and maintains effective rapport with clients; engages clients in services and responds and coordinates appropriate referrals in a timely manner.

Insures the provision of clinical interventions, utilizing flexibility in approach, maintenance of appropriate boundaries, and sound psychotherapeutic principles.

Insures that all required clinical and administrative paperwork is completed within required time frames.

Adheres to Organization policies, professional standards and legal requirements regarding clinical practice.

Insures that monthly peer reviews are completed

Provides Direct Services to clients as required.

Maintain confidentiality in compliance with HIPAA privacy regulations and Maintains security of both electronic and paper client documents.

Develop and demonstrate cultural competency in service provision to the Diversity of persons served.

Ensure that clients with substance abuse/dependency diagnosis are appropriately placed under the ADAM (American Society of Addiction Medicine) patient placement criteria and in accordance with Florida Administrative Code 65b.30.

 

Bachelor's degree in the Human Services field with a minimum of two years' experience with the community mental health or a Masters Degree in the Human Services Field.   

Minimum four years relevant clinical experience. 

 Minimum two years supervisory experience preferred. 

Community-based experience preferred.
