SOAR Coordinator

This is a professional position providing integral support and leadership in the development of the county-wide Supplemental Security Income/Social Disability Income (SSI/SSDI) Outreach, Access and Recovery (SOAR) benefits program. The SOAR coordinator will convene and lead the local SOAR support meetings to advise and encourage those working with people experiencing homelessness in preparing and submitting complete and appropriate applications.

DUTIES

  • Direct Services
    • Serve as the community coordinator for county-wide implementation of SOAR.
    • Conduct meetings with community partners to discuss SOAR implementation.
    • Develop relationships with the office of SSI/SSDI to expedite the processing of SSI/SSDI applications.
    • Edit and support SSI and SSDI benefits applications prepared by anyone in the county submitted for people experiencing homelessness.
    • Serve as a liaison between people submitting SSI/SSDI applications and the Social Security Administration offices.
    • Develop and maintain a positive, professional relationship with the community partners.
    • Conduct Outreach to engage individuals in need of services and identify potential SOAR candidates.
    • Conduct interviews with clients to gather information to complete SSI/SSDI applications.
    • Gather medical records and other information to complete SSI/SSDI applications.
    • Consult with team members on SOAR strategies.
    • Maintain records in the Electronic Health Record, including progress notes, incident reports, referrals, and bio-psychosocial evaluations.
    • Transfer and/or close clients in a professional manner.
    • Adhere to Organization policies, professional/ethical standards and legal requirements regarding clinical practice.
    • Plan and implement educational groups.
    • Transport clients as needed and required in personal vehicle as part of service provision.
    • Visit or work at sites outside the office as needed or required for services provision and provide own safe transportation to such locations.
    • Coordinate regular SOAR Meetings, at least bi-monthly with all SOAR-trained stakeholders.
    • Participate in the SOAR - HLB Steering committee
    • Seek Co-location when feasible and when co-location enhances efforts.
    • Assist in the implementation of the centralized SOAR tracking system.
    • Provide regular SOAR status reports.

QUALIFICATIONS

  • Bachelor's degree in Psychology, Social Work or related field required, with two years' experience preferred.
