SOAR Case Manager/Benefits Specialist

This is a composite position providing case management, supportive interventions, and assistance with benefit enrollment to individuals and families who are homeless. Work involves outreach, access, and recovery services to individuals experiencing homelessness and assisting them with the SSI/SSDI application for benefits. Services are provided in the community.

DUTIES

  • Clinical Direct Services
    • Establish and maintain effective rapport with consumers.
    • Work with the consumer to obtain documentation required for the benefits application.
    • Work effectively and in collaboration with other service agencies, funders and consumer groups.
    • Develop realistic and effective screening tools to assist with SSI/SSDI eligibility.
    • Follow community implementation for SOAR.
    • Edit and support SSI and SSDI benefits applications prepared by anyone in the county submitted for people experiencing homelessness.
    • Develop and maintain a positive, professional relationship with the community partners.
    • Conduct Outreach to engage individuals in need of services and identify potential SOAR candidates.
    • Conduct interviews with clients to gather information to complete SSI/SSDI applications.
    • Gather medical records and other information to complete SSI/SSDI applications.
    • Consult with team members on SOAR strategies.
    • Work in partnership with the SSI/SSDI office regarding eligibility for consumers.
    • Maintain continuity of contact with consumers as appropriate regarding their applications.
    • Maintain appropriate professional boundaries with consumers.
    • Involve significant others of consumers in the activities as appropriate.
    • Exhibit flexibility regarding clinical approach in order to most effectively respond to differing consumer problems.
    • Transfer and/or terminate consumer cases in a professional manner.
    • Adhere to Organization policies, professional/ethical standards and legal requirements regarding clinical practice.
    • Plan and implement educational groups regarding SOAR.
    • Transport clients as needed and required in personal vehicle as part of service provisions.
    • Visit or work at sites outside the office as needed or required for services provision and provide own safe transportation to such locations.
    • Prepare benefits application and submit to Social Security and/or Medicaid.
    • Assist client with appeals if denied.
    • Research, preparing reports, negotiation with the office of Social Security and/or judicial staff to resolve cases.
    • Submit court documents if required and attend court hearings if necessary to advocate for client.

QUALIFICATIONS

  • Bachelor's degree in Psychology, Social Work or related field required, with one year experience preferred.
