PT/FT Family Support Coordinator

This individual is responsible for providing on-site, individual and family intervention to children, their families, foster families, and primary caregiver(s). This individual works as a team member with a case manager, foster parents, primary care giver and other appropriate professionals. The Family Support Coordinator provides assistance, intervention and education which is focused upon resolving issues of abuse and neglect by assisting with the acquisition of services related to issues such as substance abuse, adjustment and relationship issues, stress, daily living skills, budget training and employment assistance. These services are provided to the identified child, siblings, birth parents, relatives, foster parents and primary caregiver(s), as appropriate.

DUTIES

• Complete case notes on each case, including participation , demonstrated change, and other related and/or medical record/case file documentation within the required time limits according to corporate or CBC policy and procedure (usually within 48 hours of occurrence).
• Provide Case Manager with the necessary verbal and written information to present at Court, IDT staffing, CPT staffngs, school staffings, discharge staffings and planning, including information relative to the completion of the visitation plan, etc. within 48 hours.
• Attend and participate in court proceedings, as appropriate and/or when required (subpoena, direct request, and when SAO determines if necessary).
• Attend and participate in staffing reviews, i.e., IDT staffings, CPT staffings, school staffings, treatment/service plan staffings and reviews, supervisory meetings, as appropriate, and according to corporate and or/CBC policy and procedure.
• Provide, strengthen and maintain a mentor support role by providing support, birth parents, primary caregiver(s) and other team members and professionals involved in the case.
• Provide on-going support to the foster placement, verifying that the family is providing a safe and secure home environment for the foster child including the provision of crisisIntervention and supportive mentoring to the primary caregiver in order to stabilize the child in their current placement.
• Attempt to create, strengthen and maintain positive, supportive working relationships between the foster parents, biological parents, all professionals involved and the child.
• Abide by the CBC Project Incident Reporting Procedure and corporate Incident Reporting procedures, as appropriate.
• Maintain appropriate documentation and concurrent planning in accordance with administrative regulation and according to corporate and or CBC policy and procedure, and complete all documentation within 48 hours for FSFN and 24 hours for AVATAR.
• Intervene and assist Directions/CBC staff in crisis situations, as necessary.Follow policies and procedures of corporation and CBC.
• Attend and participate in appropriate training for professional growth as developed by the CBC and Directions as required by the State of Florida and according to corporatepolicy and procedure. A minimum of 15 hours is required annually.
• Transport clients as assigned using personal vehicle.

QUALIFICATIONS

• Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
• Knowledge of child welfare, early childhood or children's mental health system preferred.
• Requires computer literacy.
