Outreach Case Manager

Street Outreach- Dispatchable/Targeted Co Response Teams (HOME).... Pay Rate: $24.04/hour ($50,000/year)

This is a professional outreach position that will provide supportive interventions to individuals over the age of 62 who are literally homeless. Work involves Outreach, VI-SPDAT evaluation, and facilitating access to emergency shelter, case management services, and community resources. Individuals served by Street Outreach teams are provided outreach and engagement efforts and services, aimed to increase overall quality of life and improving health outcomes by addressing social determinants of health of individuals who are living street homeless. In addition, Street Outreach will ensure individuals who are uninsured are enrolled in the Pinellas Health Plan.

This position will work closely with other service providers, law enforcement, emergency medical services providers, fire rescue, and code enforcement to provide comprehensive services in the community. Street Outreach services are intended to provide multiple touches to the same individual(s) until the individual that is literally homeless is engaged and sheltered. Once the literally homeless individual is sheltered all Street Outreach contacts end. The street outreach team works to link all other individuals with temporary emergency shelter, or other permanent housing options such as relocation to family members or friends and other needed resources to ensure the overall well-being of the people served. On occasion, and when no other temporary emergency shelter is available Street Outreach Teams will prioritize high risk vulnerable populations for placement in paid emergency shelter including hotels. The work schedule and hours will be flexible, to include evening work if and when required at the discretion of the Supervisor of Street Outreach.

EDUCATION EXPERIENCE/REQUIREMENTS:

Bachelor's degree in Psychology, Social Work, criminal justice or related field required, with a minimum of five years of previous experience in outreach, crisis intervention, case management and supportive interventions required. Previous experience working with vulnerable populations to include people living homeless, people living with a mental illness, people involved in the criminal justice system, or families involved in the child welfare system is required for this position. Any of the following certifications will be accepted and count toward 4 years of experience: MCAP, CAP, CBHCM, CBHCMS, CMHP, CWCM, CWPI with 1 year of case management experience. Hours will be flexible, to include evening and weekend work will be required at the discretion of the Street Outreach Supervisor.
