Intensive Case Manager

Staff in Together We Rise will work specifically with children and their families who have come to the attention of the child protective investigation division of the Pinellas County Sheriff's Office, and who have been determined to be safe but at high risk of future child abuse, and neglect. Together We Rise (TWR) will work with all members of the family with a team based approach in a supportive, empowering and respectful manner to assist the family with creating safe and stable parenting practices to facilitate long-term behavior change that will ensure that children are safe in their home.


Requirements of the Intensive Case Manger position include assessing, linking, coordinating and monitoring services related to all social determinants of health to include mental health, social and educational aspects, safe housing, community connections and providing those services within the home or in the community. The Intensive Case Manager is focused on the "here and now" and works in a strengths based, honest, and direct approach, collaborating with the parents to address barriers to include housing stability, connection to health plans, time management, parenting skills and enhancing protective capacities. The intensive case manager will be an active participant at the Integrated Decision Team (IDT) process and will provide face to face visits with the family according to the frequency required in order to mitigate risk of future abuse or neglect of the child(ren). This is a field based position and all services must be provided face to face. There is no opportunity for the provision of services from a remote location and services are not provided virtually. This position is required to complete dual entries into two separate systems: the agency electronic health record and the Florida Safe Families Network (FSFN). DFL prides itself on the fact that it is a trauma informed care services provider. This position requires becoming and maintaining the following certifications via Florida Certification Board: BHCM certification within 3 months of hire; and High Fidelity Wraparound Facilitator certification within 6 months of hire.


EDUCATION EXPERIENCE/REQUIREMENTS:

  • Must hold at least a Bachelor's Degree Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study a minimum of 2 years of experience working with at-risk populations to include families experiencing substance misuse, domestic violence, mental health, and emotionally disturbed children.
  • Previous child welfare experience and certification is strongly preferred.
  • Must acquire knowledge of community resources
  • Knowledge of child development, including physical, mental, emotional and cultural issues preferred is required.
  • Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly.
  • Must be vaccinated against COVID-19 or be willing to be vaccinated against COVID-19 immediately upon hire. This is a condition of employment, exemptions to this requirement must be addressed in advance with HR.
  • Must become and maintain BHCM certification within 3 months of hire.
  • Must become and maintain High Fidelity Wraparound facilitator certification 6 months of hire.
  • Must maintain the BHCM and HFW certification throughout employment at the employees personal expense.
