Intensive Case Manager

POSITION SUMMARY:

This position entails working with families using a team approach to increase the potential for the timely and safe achievement of permanency for children being served primarily in the child welfare system. This position entails works with the child safety dependency case manager and other entities to stabilize children's placements in both out of home care and in home care. This position will also have a shared goal of reducing the potential for removal of children from the home. It includes assessing, linking, coordinating and monitoring services from mental health, social and educational aspects. Collaborate and develop service plans in partnership with the family. Work closely with the family (both adults and children) to identify services needed in the home to maintain the least restrictive environment. Services are provided in-home or in the community. Work closely with the family to connect with resources and offer educational, financial or other assistance as needed or appropriate. Attend Integrated Decision Team Meetings. Transport clients in personal vehicle as part of service provision. Complete documentation in a timely manner relative to services provided. Handle crisis situations in a professional and timely manner and apprises supervisor. Based upon the risk factors, the Family Care Manager will fulfill more frequent Weekly visits. Intensive Family Care Manager will visit the family a minimum of once a week up to seven times a week.-. For families identified as High Risk, the Intensive Family Care Manager will offer to visit at least four weekly visits. Responsible to meet productivity requirements of 60% which includes 1152 billable hours annually or a minimum of 24 billable hours per week.

Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees may be required to work in the field, be "on call" and/or be available and able to work remotely providing telehealth services depending on the needs of the individual being served.

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Must hold at least a Bachelor's Degree Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study.
  2. Experience with child welfare, early childhood education or children's mental health preferred.
  3. Must acquire knowledge of community resources
  4. Knowledge of child development, including physical, mental, emotional and cultural issues preferred.
  5. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly.
