Intensive Case Manager

Intensive Case Managers I- Responsible for working with individuals and/or families using a team approach to decrease homelessness and achieve stable and permanent housing potential.  It includes assessing, linking, coordinating and monitoring services from mental health, social and educational aspects. Collaborate and develop service plans in partnership with the individuals and/or families. Work closely with the individuals and/or families to identify services needed in the home to maintain in a stable environment.  Services are provided in-home or in the community.  Work closely with the individuals and/or families to connect with resources and offer educational, financial or other assistance as needed or appropriate.  Transport clients in personal vehicle as part of service provision. Complete documentation in a timely manner relative to services provided.  Handle crisis situations in a professional and timely manner and apprises supervisor. This position will work closely with members of Project HOPE such as: Street Outreach & Engagement Specialist, Crisis Counselor, IDT Facilitator and collaborate with other community  service providers externally to provide complete comprehensive services.   This position will be primarily field based.  Hours will be flexible, to include evening and weekend work will be required at the discretion of the Homeless Services Supervisor. Must hold a Bachelor's Degree Social Work or related area of study from an accredited university with two years' experience working with homeless and/or vulnerable populations such as: families with minor children, persons aged 60 years and older or persons struggling with SPMI / SUD preferred.   Must acquire knowledge of community resources

Directions for Living is an essential services provider.   All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees may be required to work in the field, be "on call"  and/or be available and able to work remotely providing telehealth services depending on the needs of the individual being served.
