POSITION SUMMARY:

This is a professional position that will provide High Risk Rapid Rehousing (HR RRH) Intensive Case Management (ICM) services, wraparound facilitation, and supportive interventions to extremely high risk families who are literally homeless and assigned to DFL as the Rapid Rehousing Services Provider from the Pinellas Coordinated Entry System (CES) list. Upon assignment from the CES list the High Risk Rapid Rehousing ICM III will immediately engage with the family to develop a permanent housing plan facilitate initial evaluations, and immediately begin to facilitate placement into permanent affordable housing. Simultaneously to the High Risk Rapid Rehousing ICM III facilitating placement into permanent affordable housing the HR-RRH ICM III is responsible for providing on- going intensive case management services such as assessing for other support needs such as mental health services, daycare, transportation to needed service appointments, earned and unearned income, etc. while maintaining the first priority of moving the individual/family into sustainable, affordable permanent housing. Families served in HR RRH are served by the ICM III for approximately 12 months of financial and case management assistance. The HR RRH ICM III is responsible for ensuring that their assigned families are permanently housed as quickly as possible and remain housed throughout the program by anticipating all of the barriers to remaining housed, and quickly identifying the most appropriate interventions to addressing the identified barriers. The High Risk Rapid Rehousing ICM III links individual/families with community resources, and provides supportive counseling. This position will work closely with other service providers, both internally and externally, and funders to provide complete comprehensive services in the community setting. Caseloads are generally 10 families with VIF-SPDAT score of 11-12 or a family with any VIF-SPDAT score who also meets one of the following conditions: a child under one, a child under the age of five, or three or more children. Each assigned family must receive a minimum of 3 hours of services per week. This position is required to complete dual entries into two separate systems: the agency electronic health record and the Pinellas Health Management Information System (P-HMIS). The High Risk Rapid Rehousing ICM III is required to attend and participate in regular IDT staffings to review the housing plan and to address all identified barriers to securing and or maintaining permanent, safe and affordable housing. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. This position is field based. Hours are flexible, evening and weekend work is required. The High Risk Rapid Rehousing ICM III will be required to become a high Fidelity Wraparound Facilitator within 6 months of accepting the position.

Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees are required to work in the field, and be "on call" and respond in a time frame outlined in policy.

  1. Bachelor's Degree in psychology, social work, criminal justice or related area of study from an accredited university strongly preferred, with a minimum of 5 years of experience working with in the field with people who are homeless or housing unstable, or other vulnerable populations required.
  2. Any of the following certifications will be accepted and count toward 4 years of the 5 year minimum experience requirement: MCAP, CAP, CBHCM, CBHCMS, CMPH, CWCM, CWCM-S CWPI, CWPI-S, CCPP.
  3. Seven (7) plus years of documentable, proven experience in field based case management, outreach, crisis intervention, and supportive services to highly vulnerable people may be substituted for educational requirement. Must acquire knowledge of community resources.
  4. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must have the ability to learn the PHMIS within the first 30 days in the position. This is a condition of employment.
  5. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
  6. Must be vaccinated against COVID-19 or be willing to be vaccinated against COVID-19 immediately upon hire. This is a condition of employment.
