Intensive Case Manager -Street Medicine

Intensive Case Manager Street Medicine The pay rate for this position is $25.38 per hour.

The Street Medicine Team is a pilot program in partnership between the Pinellas County Department of Health (DOH) and Directions for Living (DFL) to provide psychiatric/medical and behavioral health case management, care coordination and system navigation services to people who are street homeless and who are either referred to or come to the attention of the Street Medicine team. The Street Medicine Team will provide targeted outreach to individuals who are unsheltered homeless living on the streets, those who frequent soup kitchens and other feeding sites, and those who have recently been discharged from a hospital or emergency shelter and have continuing primary and or behavioral health needs. The Street Medicine team shall consist of one (1) Intensive Case Manager (ICM) employed by DFL, one (1) Department of Health Nurse, one (1) Department of Health Administrative Support worker, one (1) Department of Health Supervisor/Manager and one (1) Department of Health PA. The Street Medicine team is a 40-hour a week position and is paired with a team of health care professionals. The Street Outreach Team will ride together in an identified Department of Health vehicle.


Street Medical Outreach services are intended to provide multiple touches to the same individual(s) until the individual or family that is literally homeless is connected to ongoing medical care and is engaged in services to alleviate homelessness and address the social determinants of health. Once the literally homeless individual or family is connected to ongoing support all Street Medicine Outreach contacts end. The DFL ICM will serve as the street guide and assist with gathering the information needed for targeted outreach to the unsheltered homeless. Additionally, the ICM will focus on ensuring that individuals are enrolled in the Pinellas County Health Plan (blue card) and assess the individual for unmet behavioral health needs to include mental health counseling, substance misuse treatment, and psychotropic medication management. . The Department of Health team members will assess medical interventions needed on a three tiered system of triage to include but not limited to: providing health education for prevention and wellness; general primary care and referrals to specialty care as medically indicated. The Street Medicine team services are intended to provide multiple touches to the same individual(s) until the individual is connected to and engaged with a permanent medical home.

The work schedule and hours will be flexible, to include evening and weekend work if and when required at the discretion of the Supervisor. Responsible to meet productivity requirements of 1248 hours of face to face client contacts annually or a 104 hours per month.

EDUCATION EXPERIENCE/REQUIREMENTS:

Bachelor's degree in Psychology, Social Work, criminal justice or related field required, with a minimum of five years of previous experience in outreach, crisis intervention, case management and supportive interventions required. Previous experience working with vulnerable populations to include people living homeless, people living with a mental illness, people involved in the criminal justice system, or families involved in the child welfare system is required for this position. Previous life experience with homelessness is a plus. Any of the following certifications will be accepted and count toward 4 years of experience: MCAP, CAP, CBHCM, CBHCMS, CMHP, CWCM, CWPI with 1 year of case management experience.
