Intensive Case Manager- Emergency Shelter

This is a professional position that provides intensive case management, care coordination, high fidelity wraparound, and supportive interventions to families and other highly vulnerable individuals (priority populations: individuals over the age of 65, families with dependent children, young adults whom have aged out of the child welfare system, and individuals with severe and persistent mental illness) who are homeless and placed in paid hotel/motel or temporary emergency shelter by street outreach teams or other service providers who have identified highly vulnerable individuals or families with no place to sleep due to homelessness. Intensive Case Management is a comprehensive approach to housing problem solving and serves individuals and families who are homeless and with the greatest service needs. This position provides outreach, assessment, planning, linkage, monitoring, advocacy, coordination and direct service provision to include assisting the individual/family with becoming document ready for rehousing. The Intensive Case Management model prescribes more intensive services, more frequent client contact and smaller caseloads.

The ICM III Emergency Shelter develops individualized housing plans, facilitates initial evaluations, and monitors the delivery of services to assist the individual/family with moving from paid temporary emergency shelter to either emergency shelter, transitional housing such as a family member or friend, or to facilitate placement into permanent affordable housing. The ICM provides on-going intensive case management services such as brief mental health services, connections to daycare, transportation to needed service appointments, etc. while working to support the individual/family with securing stable, affordable housing. The Intensive Case Manager links individual/families with community resources, provides supportive counseling, prevention and intervention practices. This position works closely with other service providers, both internally to DFL and externally, and funders to provide complete comprehensive services in the community setting. The ICM is required to attend and participate in regular IDT staffing's to review the housing plan and to address all identified barriers to securing permanent, safe and affordable housing. Individuals and Families receive ICM services until they become housed or are assigned to a formal rapid re-housing provider. All ICM's work as a part of a team that is accessible 24 hours per day, 7 days per week. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. This position is field based. Hours are flexible, evening and weekend work is required. Caseload size is 1:15. The ICM will be required to become a High Fidelity Wraparound Facilitator within 6 months of accepting the position.

EDUCATION EXPERIENCE/REQUIREMENTS:

1. Must hold a Bachelor's Degree Social Work or related area of study from an accredited university
2. Five (5) years' experience working with the homeless population and/or vulnerable children and families preferred
3. Must acquire knowledge of community resources
4. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must be able to use Telehealth platform for service delivery.
5. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
