Intensive Case Manager- Adult/SPMI

This is a professional position with the Pinellas County Empowerment Team 2.0 (PCET) that will provide Intensive Case Management (ICM) services, supportive interventions, and ongoing assertive engagement to a subacute population of adults with severe or persistent mental illness (SPMI) who are disproportionately overinvolved with the County Jail, and the public crisis stabilization unit/receiving facility and are at an increased risk for repeated or overuse of public resources. The adults will be identified exclusively by the Pinellas County Human Services and referred to PCET 2.0. As a member of the team, the ICM provides a higher level of intensity and frequency of services based on the clinical and psychosocial indications of the individual. The level of frequency is based on the acuity/need of the individual and upon entering the program a client will start at the highest level and services are delivered in a variety of settings, including outside the agency office (i.e., home, street, and restaurant). The ICM shall deliver approximately three hours each with each client per week, resulting in an average of nine service hours per client per week. Clients shall have a minimum of 3-5 home/client visits per week, with a minimum of 50 percent of these contacts conducted outside the agency office and in the community.

The ICM provides intensive case management services to include initial needs assessments, development of housing plans and comprehensive service plans, and immediate assessment and linkage to all other needed services to ensure the participant achieves and maintains stabilization while preventing crisis. The ICM is identified as the person that works hand-in-hand with the client to develop a plan of care that focuses on the client voice and choice by ensuring the individual has access to necessary people and resources and that they have ownership of the planning process. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. Hours are flexible, evening and weekend work is required. The ICM will be required to become TCM Certified (CBHCM) within three months of accepting the position.

Education/ Experience Requirement:

  • Bachelor's Degree in Social Work or a related area of study from an accredited university
  • Five years of experience working with the homeless population and/or persons struggling with severe and persistent mental illness and/or substance misuse disorders
