Case Manager

This position entails working with families using a team approach to increase the potential for the timely and safe achievement of permanency for children being served primarily in the child welfare system. This position entails works with the child safety dependency case manager and other entities to stabilize children's placements in both out of home care and in home care. This position will also have a shared goal of reducing the potential for removal of children from the home. It includes assessing, linking, coordinating and monitoring services from mental health, social and educational aspects. Collaborate and develop service plans in partnership with the family. Work closely with the family (both adults and children) to identify services needed in the home to maintain the least restrictive environment. Services are provided in-home or in the community. Work closely with the family to connect with resources and offer educational, financial or other assistance as needed or appropriate

Targeted Case Management includes assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, entitlement, and vocational rehabilitation to help children, families and adults live work, and participate fully in their community. It includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual, which reflects strengths and self identified goals. As an integral part of case management services, discharge/transition services are arranged or coordinated by the case manager as an individual is preparing for discharge and transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training or school attendance.)   All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs. Attend Integrated Decision Team Meetings. Transport clients in personal vehicle as part of service provision. The Case Manager will be assigned to be on call for coverage after hours and on weekends. Staff are required to complete all corporate and industry related training for child welfare services, including the principles and techniques of  signs of safety.    Responsible to meet productivity requirements of 72% which includes 1326 billable hours annually or a minimum of 29 billable hours per week.

EDUCATION EXPERIENCE/REQUIREMENTS:

Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness. Completion of Targeted Case Management Certification required. Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team. Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.

Hold or obtain child welfare certification.
