Care Coordinator

Rate of Pay: $23.08/hour

This is a Pilot project between the Pinellas County Department of Health (DOH) and Directions for Living to provide care coordination to individuals who have been referred for behavioral health services and who are not engaged or sporadic in attendance are at needed behavioral health or medical appointments. The Care Coordinator is responsible for locating the individuals, developing rapport and reducing barriers to ensure their consistent engagement with Pinellas County Health Plan and their Health Care Home. The Care Coordinator will be responsible to work in partnership with the Department of Health Medical Homes and/or the Healthcare for the Homeless Program. Care Coordinators deliver services in the field, home, community, clinic or other locations. Services such as, outreach/ rapport building, providing referrals to community partners to address basic needs, linkages to mental health and medical services and assisting in scheduling appointments. Care Coordinators will address identified barriers to attend services and work alongside the individual and their informal supports to create a plan that is achievable.

Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees will be required to work in the field, and or be "on call" depending on the needs of the community during the time of an emergency.

EDUCATION EXPERIENCE/REQUIREMENTS:

Bachelor's degree in Psychology, Social Work, criminal justice or related field required, with a minimum of two years of previous experience in care coordination, case management and supportive interventions required. Any of the following certifications will be accepted and count toward 1 year of experience: MCAP, CAP, CBHCM, CBHCMS, CMHP, CWCM, CWPI with 1 year of case management experience.

Must acquire knowledge of community resources. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must be able to document in to two separate systems on the same day as service was delivered
