Care Coordinator

POSITION SUMMARY:

Mental Health Care Coordinator will enhance the delivery of treatment services and recovery supports and to improve among the identified priority population. Mental Health Care Coordinator serves to assist individuals who are not effectively connected with services and supports they need to transition successfully from higher levels of care to effective community based care. This includes services and supports that affect a person's over all wellbeing, such as primary physical health, housing, and social connectedness. Care Coordinator will connect systems including behavioral health, primary care, peer and natural supports, housing, education, vocation and the just system. It is time limited, with a heavy concertation on educating and empowering the person served and provides a single point of contact until a person is adequately connected to the care that meets their needs.

It includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual, which reflects strengths and self identified goals. As an integral part of Mental Health Care Coordination services, discharge/transition services are arranged or coordinated by the Mental Health Care Coordinator as an individual is preparing for discharge and/or transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training and/or stable mental and physical health.)   All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs. Responsible to meet productivity requirements of 75% which includes 1440 billable hours annually or a minimum of 30 billable hours per week.

Directions for Living is an essential services provider.   All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees may be required to work in the field, be "on call" and/or be available and able to work remotely providing telehealth services depending on the needs of the individual being served.   

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
