Behavioral Health Case Manager

Rate of Pay: $23.08/ hour

POSITION SUMMARY: Case Management includes assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, entitlement, and vocational rehabilitation to help adults live work, and participate fully in their community. As an integral part of case management services, discharge/transition services are arranged or coordinated by the case manager as an individual is preparing for discharge and transition from one level of care to another. These services are designed to support the attainment of individual-defined goals All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs.

Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation. Essential employees may be required to work in the field, be "on call" depending on the needs of the individual being served.

EDUCATION EXPERIENCE/REQUIREMENTS:

1. Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
2. Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
3. Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.
