211 Homelessness Prevention Referral Coordinator

POSITION SUMMARY: $18.26/hour

This is an administrative position that provides initial screening and housing needs assessment to prospective targeted prevention individuals/families that have been referred to Directions for Living by 211. The 211 Referral Coordinator answers a dedicated phone line from 211 Tampa Bay Cares and receives information on individuals/families who are currently housed in one of 5 identified zip codes (33712, 33755, 33760, 33705, 33711) and who have called 211 Tampa Bay Cares to report that they are at risk of homelessness due to unpaid rents, past due utilities, or landlord tenant disputes. The 211 Referral Coordinator will continually track all incoming referrals from 211 Tampa Bay Cares and is responsible for contacting the individual/family within 24 business hours of receiving the referral from 211 Tampa Bay Cares. The 211 Targeted Prevention Referral Coordinator must strive to work efficiently and quickly to ensure that the individual/family in need of homelessness prevention is contacted as quickly as possible. Timing is of the essence. Once the individual/family is determined to meet the Targeted Prevention criteria, the individual/family will be immediately assigned to a Targeted Prevention Intensive Case Manager for service provision. The 211 Referral Coordinator requires refined communication and de-escalation skills, as individuals/families at risk of homelessness may become anxious, agitated or angry when talking about their current situational challenges. The 211 Referral Coordinator must be experienced within electronic health record systems, as all information will be documented in DFL's electronic health record (Avatar) and the Pinellas Homeless Management Information System (PHMIS). This position will work closely with the Prevention and Diversion Team, and other Housing First programs. This position is primarily office based. Hours are flexible, evening work may be required. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided.

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Must hold a Bachelor's Degree Social Work or related area of study from an accredited university
  2. Five (5) years verifiable experience working with the homeless population and/or vulnerable children and families preferred.
  3. Must be able to rapidly acquire knowledge of community resources.
  4. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must have the ability to learn the PHMIS and Avatar within the first 30 days in the position. This is a condition of employment.
  5. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
