This is a professional clinical position that will work in partnership with local law enforcement personnel providing case management counseling and supportive interventions to families and individuals who are homeless or at risk of becoming homeless.  Work involves Outreach, Evaluation, facilitating access to emergency shelter, case management services, community resources, community education, and application of psychotherapeutic and prevention practices. This position requires riding with a law enforcement officer to connect to homeless families and individuals will be outreached at shelters, other service provider's locations, businesses, private homes or other location they can be found.

PRIMARY RESPONSIBILITIES:

  • Establish and maintain effective rapport with consumers, and community partners.
  • Develop accurate and complete clinical evaluations/assessments of consumers and their needs.
  • Develop realistic and effective self-sufficiency/treatment plans with consumers, and update them as appropriate.
  • For consumers receiving individual services, complete self-sufficiency/service/treatment plans within the Program's expected time frame and route to Supervisor, per Organization policy.
  • Review and update consumers' plans appropriately, according to Program expectations.
  • Maintain progress notes according to Program policies and procedures, indicating progress made toward plan goals and objectives.
  • Complete consumer transfers, closings, referrals, summaries, reports, authorization for release of information and other forms according to Organization and Program policies and procedures.

EDUCATION EXPERIENCE/REQUIREMENTS:

  • Bachelor's degree in Psychology, Social Work or related field required, with two years' experience preferred.
  • Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
